Overview
The Salt Lake County Health Department's free home-visiting nurses and educators help low-income families achieve healthier pregnancies and stronger child development, as well as provide resources and link you to services. Healthier families translate to safer, stronger communities for all of us!
Our home visiting programs include:
- Nurse-Family Partnership (for first-time mothers in SLCo who meet income guidelines; you must enroll before week 28 of your pregnancy)
- Parents as Teachers (for families in SLCo who are pregnant or have a child under 3 and who meet income guidelines)
- Targeted Case Management (for families on Medicaid with a child under 8 years old)
We also offer home visiting programs for people with asthma and children who have an elevated blood lead level.
