Religious/Charitable Refund Form

Organizations who didn't qualify for the exemption must pay the assessment fee and request a refund.

Reimbursement Refund Form

Utah State and local government representatives must pay the assessment fee at the time of purchase, then request a refund from the Salt Lake County Treasurer.

Hotel Partner Refund Form

CTAA Hotel Partners should utilize this form for requesting refund in the event of underpayment or overpayment.
