Tanning Operators

Because of the known health risks associated with tanning, Salt Lake County Health Regulation #25: Tanning Establishments requires establishments that offer indoor tanning to have a tanning operator certified by the health department on site during all hours of operation, among other requirements.

Tanning Operator Certification

Tanning operator training may take 30 minutes or more. Certifications are good for up to 3 years. The application fee is $40.

To become a certified tanning operator, complete the steps below:

Carefully review the Tanning Operator Training PDF in preparation for the certification test.

Fully complete the Tanning Operator Certification Application below, including the test within the application, and submit the completed PDF to the Sanitation and Safety Bureau.

Complete the application and test only on a computer with the current version of Adobe Reader. Do not use a mobile device or web browser PDF viewer.

After submitting your completed application, including the test, call 385-468-3835 (Monday through Friday from 8:00 a.m. to 5:00 p.m.) to pay the $40 application fee via Visa or Mastercard.

You may also print and mail your completed application, along with check or money order, to the Environmental Health Division.

We also accept cash and applications in-person at the address below.
