Request to Remove Your Signature from an Initiative Petition
Voters who sign a packet for an initiative petition and wish to remove their signature may request it in just a few steps:
- Print a copy of the PDF linked below.
- Complete and sign the physical copy.
- Return it to our office by mail or in-person.
Please note: Utah code only allows signatures to be removed from petitions with a physical signature. We cannot accept digital signatures or digital copies of this form.
