Voter Identification Requirements

If you are not already registered to vote and are voting with a provisional ballot, you will need to provide TWO acceptable forms of identification: one that verifies your identity AND one that provides proof of residence.

If you are already registered to vote in Utah, you must provide either:

ONE primary form of identification that is currently valid (not expired). Acceptable forms include:

  • Currently valid Utah driver license
  • Currently valid Utah identification card
  • Currently valid identification card issued by the state
  • Currently valid identification card issued by a branch, department, or agency of the U.S. government
  • Currently valid Utah permit to carry a concealed weapon
  • Currently valid U.S. passport
  • Currently valid U.S. military identification card

Or one of the following identification cards, with or without a photograph:

  • Valid tribal identification card
  • Bureau of Indian Affairs card
  • Tribal treaty card

OR

TWO forms of identification that, when combined, include your name and provide evidence that you reside in your voting precinct. Acceptable forms may include:

  • Certified birth certificate
  • Valid Social Security card
  • Check issued by the state or federal government within 90 days of the election
  • Paycheck from the voter's employer issued within 90 days of the election
  • Currently valid Utah hunting or fishing license
  • Certified naturalization documents (not a green card)
  • Currently valid license issued by an authorized U.S. government agency
  • Certified copy of court records showing the voter's adoption or name change
  • Valid Medicaid, Medicare, or Electronic Benefits Transfer (EBT) card
  • Bureau of Indian Affairs card
  • Tribal treaty card
  • Valid tribal identification card
  • Currently valid identification card issued by a local government within Utah
  • Currently valid identification card issued by an employer
  • Currently valid identification card issued by a college, university, technical school, or professional school located in Utah
  • Current Utah vehicle registration

Proof of U.S. Citizenship

Proof of U.S. citizenship may be required when registering to vote or if the Election Office does not already have proof of citizenship on file.

Proof of U.S. citizenship may be established by one of the following:

  • A Utah driver license or state identification number that verifies U.S. citizenship
  • A legible copy of a birth certificate that verifies U.S. citizenship
  • A legible copy of a U.S. passport that identifies you and your passport number
  • An alien registration number that verifies U.S. citizenship
  • A legible copy of U.S. naturalization documents
  • A Bureau of Indian Affairs card number, tribal treaty card number, or tribal enrollment number
  • A legible copy of a Certificate of Degree of Indian Blood or a Bureau of Indian Affairs affidavit of birth
  • Other documents or methods of proving U.S. citizenship established in accordance with federal law
